Adults with invasive pneumococcal disease: missed opportunities for vaccination.

摘要: Background The pneumococcal polysaccharide vaccine (PPV) can prevent invasive disease (IPD) in the elderly and those with certain underlying illnesses. However, uptake remains suboptimal. Identification of missed opportunities for vaccination could guide new strategies improving uptake. Missed were defined as one or more visits to a hospital, emergency room (ER), main provider 2 years before infection among unvaccinated, adult IPD case–patients indication. Methods Adults aged 18 older identified six Active Bacterial Core surveillance/Emerging Infections Program Network sites during 1-year period 2001 2003. Using chart review, patient/proxy interview, questionnaire, questionnaires from additional providers, data collected on demographics, indications, status, recent healthcare encounters. Results A total 1878 cases enrolled, 83% had Of 1177 indication sufficient information encounters, 617 (52%) unvaccinated. these, 566 (92%) vaccination, 54% hospitalized, 58% ER visits, 76% visited their illness. number providers (median=6) was higher than hospitalizations (median=1), (median=1). Conclusions One documented nearly all unvaccinated Most multiple times. Implementation systematic PPV programs outpatient settings will likely increase high-risk adults.
