Assessing and mapping language, attention and executive multidimensional deficits in stroke aphasia

摘要: There is growing awareness that aphasia following a stroke can include deficits in other cognitive functions and these are predictive of certain aspects language function, recovery rehabilitation. However, data on attentional executive (dys)functions individuals with still scarce the relationship to underlying lesions rarely explored. Accordingly this investigation, an extensive selection standardized non-verbal neuropsychological tests was administered 38 chronic post-stroke aphasia, addition detailed testing MRI. To establish core components variable patients' performance, behavioural were explored rotated principal component analyses, first separately for tests, then combined analysis including all tests. Three orthogonal extracted, which interpreted as shift-update, inhibit-generate speed. also extracted representing phonology, semantics speech quanta. Individual continuous scores each included voxel-based correlational methodology analysis, yielding significant clusters components. The shift-update associated posterior left temporo-occipital bilateral medial parietal cluster, mainly frontal regions, speed several small right-sided fronto-parieto-occipital clusters. Two complementary multivariate brain-behaviour mapping methods used, showed converging results. Together results suggest range brain regions involved attention functioning, non-language domains play role abilities patients aphasia. In conclusion, our findings confirm extend understanding multidimensionality emphasize importance assessing cognition patient group provide directions future research clinical practice.
