Die fibrovaskuläre Umwandlung der CNV bei nAMD unter lang andauernder Anti-VEGF-Therapie

摘要: Bei der neovaskularen altersabhangiger Makuladegeneration (nAMD) kann es unter lang andauernder Anti-VEGF-Therapie zu einer fibrovaskularen Umwandlung choroidalen Neovaskularisation (CNV) kommen. Es fehlen Definitionen, wie eine differenzierte Quantifizierung einhergehenden morphologischen Veranderungen erfolgen kann. Die vorliegende Pilotstudie dient dem Ziel, die hierzu am besten geeigneten Bildgebungsmodalitaten definieren. 56 Augen mit fibrosierter CNV nach min. 2 jahriger und 12 intravitrealen Anti-VEGF-Injektionen wurden folgenden hinsichtlich Abgrenzbarkeit von vaskularem fibrosem Anteil sowie assoziierter Atrophie retinalem Pigmentepithel (RPE) Unterbrechung ellipsoiden Zone (EZ) untersucht: Multicoloraufnahme (MC), Fundusautofluoreszenz (FAF), Fluoreszeinangiographie (FA) Indocyaningrunangiographie (ICGA), Spectral-domain-optische Koharenztomographie (SD-OCT) OCT-Angiographie (OCTA). Der vaskulare fibrosierten konnte mittels OCTA, fibrose SD-OCT abgegrenzt werden. RPE-Atrophie war bestmoglich FAF darstellbar, jedoch Abgrenzung auch mithilfe MC ICGA moglich. EZ-Unterbrechung im SD-OCT-b-Scan ist zur Darstellung CNV, geeignet. Mittels quantifiziert werden; b Scan, nicht transverse-structure-Scan abgrenzbar. Mithilfe OCTA Detektion des vaskularen Anteils
