Interdisciplinary Surgical Approaches in Vaginal and Perineal Reconstruction of Advanced Rectal and Anal Female Cancer Patients

摘要: Relapsing or far advanced rectal and anal cancers remain difficult to treat require interdisciplinary approaches. Due modern standard protocols all patients receive irradiation neoadjuvant chemotherapy – in case of a relapse second - rendering the surgical site prone infections oftentimes long lasting sinus septic complications after exenteration pelvis. Despite an improved overall survival rate these downside radical tumor surgery pelvis is major loss quality life, especially women when parts vagina need be resected. Derived from our experince with over 300 receiving pelvic perineal reconstruciton transpelvic vertical rectus abdominis myocutaneous (VRAM) flap we studied impact this technique on outcomes female without vaginal reconstruction following exenteration. We found out that tpVRAM reliably perfused helps reduce term wound healing desasters irradiated perineal/vaginal/gluteal region.
