Improving the outcomes of anticoagulation in rural Australia: an evaluation of pharmacist‐assisted monitoring of warfarin therapy

摘要: Summary Objective:  The aim of this project was to assess whether rural pharmacist involvement in the management patients receiving warfarin has potential lead safer and more effective anticoagulation, is valued welcomed by their general practitioners (GPs). Methods:  A convenience sample pharmacists trained use CoaguChek S International Normalized Ratio (INR) monitor then conducted pharmacy-based testing for approximately 3 months. Two types were performed pharmacy: (i) comparison defined as tests taken within 4 h conventional laboratory or (ii) additional testing, which a test with no direct taken. Pharmacists, GPs completed anonymous satisfaction surveys after completion testing. Results:  Pharmacists from 16 pharmacies monitor. During trial period, 518 INR on 137 different patients. total 120 evaluated against results testing. values significantly correlated (mean 2·32 ± 0·77 2·32 ± 0·59 respectively; r = 0·88, P < 0·0001). 398 pharmacy 8·5% resulted subsequent dosage change. monitoring well received pharmacists, patients.
