摘要: A follow up study was carried out in 1990 on 169 well documented patients initially presenting with osteoarthritis of the hands or knees between 1975 and 1977. Radiographic change knee used as outcome measure. Sixty three subjects had paired radiographs a mean 11 years apart were 69 (range 52-87) old at up. Thirty known to have died, 28 untraceable, 48 traced but did not films available. The read independently blind time sequence by two observers using five different radiological scoring methods. Most increase Kellgren Lawrence grade, only 33% deteriorating over period. results similar when subject categorised their worst knee. When more sensitive global score 50% showed slight deterioration 10% improved. Visual analogue pain scores remained unchanged. Those baseline greater chance progressing, those existing contralateral These suggest that most attending rheumatology clinics do deteriorate radiographically symptomatically an year More work is needed selection early detection poor prognosis focusing intervention this high risk group.
