Massive pulmonary embolism: treatment with full heparinization and topical low-dose streptokinase.

摘要: Three patients with massive pulmonary embolism were treated low-dose streptokinase (delivered topically via the artery) and simultaneous full-dose heparin. In two acute emboli, rapid but incomplete lysis was observed over 15-30 hours. one patient recurrent embolization three weeks, major embolus shrank considerably. Pulmonary artery pressure angiographic findings improved significantly in both emboli. old embolus, did not improve despite considerable improvement arterial PO2. These data suggest that early topical administration of plus heparin may be treatment choice for embolism, particularly those compromised cardiopulmonary status.
