摘要: Toxoplasmic optic neuropathy is rare and usually occurs mono-ocularly. This case report demonstrates a presentation of bilateral juxtapapillary retinochoroiditis (Jensen disease) due to toxoplasma infection in young healthy patient. A 20-year-old lady presented with painless blurring central vision for 5 days duration. It was preceded by fever, upper respiratory tract symptoms headache. There no history contact or being scratched cat. Visual acuity counting fingers the right eye 6/45 left eye. presence relative afferent pupillary defect Optic nerve functions were impaired bilaterally which severe Both eyes showed mild anterior segment inflammation vitritis. Fundus examination revealed swollen disc. Optical coherence tomography (OCT) intra-retinal sub-retinal fluid at macular area bilaterally. Serology anti-toxoplasma Immunoglobulin G (IgG) positive titre 1450 IU/ml. Computed scan (CT scan) brain orbit normal. diagnosis Jensen disease made. Oral azithromycin 500 mg daily guttae prednisolone 4 hourly 6 weeks commenced. 50 (1 mg/kg/day) added after completion 1 week antibiotic tapered down within weeks. improvement as early 3 post initiation treatment. Upon completing treatment, her has improved 6/7.5 on both resolution disc swelling fluid. Early recognition treatment associated result good visual prognosis.
