Suicide due to oral ingestion of lidocaine: a case report and review of the literature.

摘要: The Authors describe a rare case of suicide in 31-year-old woman, due to oral ingestion lidocaine; the histological and toxicological findings are discussed provide useful information present experience with this particular modality death. Histological examination revealed generalized stasis. In myocardium we observed segmentation myocardial cells and/or widening intercalated discs associated group hypercontracted "square" nuclei line hyperdistended ones. Non-eosinophilic bands sarcomeres alternating stretched, often apparently separated sarcomeres, small foci paradiscal contraction band necrosis, perivascular fibrosis were too. Lidocaine was detected subject's urine through immunoenzymatic screening. Toxicological analysis by solid-liquid extraction gas chromatography-mass spectrometry (GC-MS) analysis, carried out identify quantify individual substances biological fluids organs. concentrations as follows: blood 31 microg/mL, gastric content 2.5 g, liver 10 microg/g, kidney 12 brain 9 spleen 24 lung 84 heart bile 6 microg/mL. No other drugs or alcohol detected. When lidocaine reaches toxic levels, serious symptoms central nervous system cardiac noted. overdose produces death from ventricular fibrillation arrest. case, according macroscopic microscopic findings, cause most likely possibly related fibrillation.
