摘要: Background: The isolation of a pathogen is vital in the diagnosis and treatment device infection. A swab culture, despite poor sensitivity, most common method used specimen collection. Objective: To determine relative value tissue cultures patients with implantable cardiac pacemakers defibrillators. Design: Prospective patient cohort study. Setting: 1,000-bed tertiary referral center Cleveland, Ohio. Patients: Consecutive pacemaker or defibrillator presenting for lead extraction from October 1, 2000 to March 31, 2001. Methods: Tissue were prospectively collected during surgeries that required extraction. Clinical manifestations, microbiology, echocardiographic data recorded without clinical system infection. Results: Seventy-one (n = 49, 69%), 18, 25%), both devices 4, 6%) requiring had pocket analysis. Infection was evident clinically 35 (49%) absent remainder. bacteria isolated coagulase-negative Staphylococcus (37%) aureus (10%). Patients infection positive more frequently (P 0.002) by culture 24, 69%) than 11, 31%). However, infections at similar rates 10, 28%) 8, 22%; P 0.48). not treated other perioperative antibiotics, did develop infections. Conclusion: Pocket are effective identification infectious pathogens infections. Positive absence signs symptoms does imply need specific therapy.
