The role of thoracic ultrasonography in the diagnosis of pulmonary embolism.

摘要: Objectives: The diagnosis of pulmonary embolism (PE) is still a problem especially at emergency units. purpose study was to determine the diagnostic accuracy thoracic ultrasonography (TUS) in patients with PE. Methods: In this prospective study, 50 suspected PE were evaluated Department Pulmonary Diseases Training and Reasearch Hospital between January 2010 July 2011. At begining, TUS performed by chest physician, subsequently for definitive computed tomography angiography all cases as reference method. Other procedures examination serum d-dimer levels, echocardiography, venous doppler legs. Both physician radiologist blinded results other Diagnosis suggested if least one typical pleural-based/subpleural wedge-shaped or round hypoechoic lesion without pleural effusion reported TUS. Presence pure normal sonographic findings accepted negative Results: diagnosed 30 patients. It shown that true positive 27 false eight 12 three. Sensitivity, specificity, predictive value, clinically 90%, 60%, 77.1%, 80%, 78%, respectively. Conclusions: high sensitivity accuracy, noninvasive, widely available, cost-effective method which can be rapidly performed. A cannot rule out certainty, but moderate/high suspicion may prove valuable tool bedside setting, critically ill immobile patients, facilitating immediate treatment decision.
