摘要: There are few studies that compare the deep inferior epigastric artery perforator (DIEP) flap to pedicled transverse rectus abdominis myocutaneous (pTRAM) for use in reconstructive breast surgery. The authors examined four factors aid decision-making: donor-site morbidity, need surgery related abdominal operative time, and complications. This is a retrospective review of patients undergoing reconstruction using DIEP or pTRAM at University British Columbia between 2002 2013. compared time abdomen- flap-related complications both groups.Reconstruction was performed 507 patients; 25.6 percent received flaps (n = 183 breasts) 74.4 underwent 444 breasts). Pedicled TRAM were more likely require closure with mesh (44.2 versus 8.1 percent; p < 0.001); 21.2 them had postoperative bulge and/or hernia 3.1 12.7 required hernia/bulge. Controlling confounders, there five times odds hernia/bulge group. 234 minutes longer than surgery. The benefits may be offset by correct wall complications. lower donor but increased time. A cost analysis needed determine most economical procedure.Therapeutic, III.
