摘要: Ensuring the safe transition of patients from hospitals to skilled nursing facilities and back hospital or community can present significant challenges. The University Michigan Health System was able overcome many these challenges through implementation a health system associated Subacute Care Service that consists geriatricians nurse practitioners working in privately operated our primary market area. We describe planning process surrounding development report on efforts date.
