摘要: Transition of care from pediatric to adult-oriented health providers is difficult for children with special needs. Children who have received solid organ transplants and their experience the same difficulties frustrations as other major illnesses. A consensus conference was organized by several transplant organizations identify issues in this area recommend possible approaches easing process transition recipients. This report summarizes discussions recommendations.
