Cancer care coordinators: realising the potential for improving the patient journey

摘要: The cancer care coordinator role is a rapidly emerging one with mandate to achieve some potentially far reaching reforms systems of care. To ensure these developments realise their potential, the author considers most effective ways design and implement models coordination that improvements being sought for Australian system.
