摘要: Summary Background  Extramammary Paget disease is an uncommon skin tumour occurring mostly in the genitoperineal region. Previous reports have shown frequent expression of androgen receptor, suggesting a tumour-proliferative effect androgens on cells. Androgen-converting enzymes such as 5α-reductase, which locally produces bioactive androgen, recently gained attention studies intratumoral actions androgens. Objectives  We investigated correlations between androgenic microenvironment and invasiveness extramammary disease, particularly terms sex differences. Methods  examined 58 cases (32 men, 26 women; 42 noninvasive, 16 invasive) using immunohistochemistry for receptor 5α-reductase. Results  In all cases, rates were 57% 55% with 38% double-positivity 5α-reductase. Only 5α-reductase rate was significantly higher invasive (81%) than noninvasive (45%; P = 0·042). For numbers double-positive results men (70%) women (17%; P = 0·039). Conclusions  Double positivity cells suggests autocrine synthesis disease. The different hormonal microenvironments male female levels affect
