摘要: Sir,A 30-year-old male presented with roughness, ridging, flattening, and opacity of all the fingernails six toenails since last two years [Figure 1]. In beginning, these changes appeared only in fingernails; then other nails became involved degree involvement increased severity. On detailed examination patient, multiple violaceous flat papules plaques were found on left side his trunk a dermatomal distribution 2]. Similar lesions not anywhere else body. Oral genital mucosa normal. According to him, around 3 months back. Routine hematological biochemical examinations within normal limits. A skin biopsy done from one showed basal cell degeneration, interface lymphocytic infiltrate, melanin incontinence. Therefore diagnosis segmental or zosteriform lichen planus association 20-nail dystrophy was made 3]. The patient started systemic corticosteroids antihistamines.Twenty-nail is characterized by spectrum nail plate abnormalities that leads roughness trachyonychia. It can be idiopathic may associated planus, psoriasis, alopecia areata, ichthyosis vulgaris, eczema, vitiligo, primary biliary cirrhosis, IgA deficiency, graft-versus-host disease.
