Evaluation of the MycoDot test in patients with suspected tuberculosis in a field setting in Tanzania.

摘要: CADRE : Des methodes rapides, faciles a pratiquer et de faible cout sont indispensables pour ameliorer le diagnostic la tuberculose dans les pays revenus, en particulier patients dont expectorations negatives l'examen bacilloscopique. Le test Mycodot, qui est base sur detection d'anticorps vis du lipoarabinomannan (LAM), doue ces caracteristiques. OBJECTIF: Apprecicr l'utilite Mycodot depistage des suspects Mwanza Tanzanie. METHODES: Les ambulatoires, se presentant une consultation un grand hopital urbain accusant symptomes suggestifs ont subi examen physique microscopique l'expectoration recherche bacilles acido-resistants. A cote cette evaluation diagnostique, l'on pratique cutane PPD sang complet. bacilloscopies etaient ete traites pendant 10 jours par l'erythro-mycine, apres quoi nouvelles pratiquees chez sujets restant symptomatiques. cliches thoraciques pris symptomatiques negatifs Tous echantillons d'expectorations mis culture mycobacteries. atteints d'une negative ainsi que ceux sans revus deux mois. L'on independamment deuxieme serologie VIH serum avait preleve au moment premiere visite. RESULTATS: Nous avons evalue total 241 tuberculose. s'est avere positif 39 (26%) bacilloscopie positives, 1 15 (7%) mais positive, 2 29 radiologique culture, 6 32 (19%) ne repondaient pas definition cas adoptee l'etude, enfin 16 102 (16%) suiets L'examen incomplet 24 patients. La sensibilite fut 13/83 soit 16%, sa specificite 86/102 84% valeur predictive positive 13/29 45%. plus elevee (41%) seronegatifs basse (3%) seropositifs VIH. concordance entre tests 80%; trouve toutefois moins reactions positives lors test. CONCLUSION: n'est utile Tanzanie ni d'autres d'Afrique sub-Saharienne, ou l'infection prevalante.
