Rising Obesity Prevalence and Weight Gain Among Adults Starting Antiretroviral Therapy in the United States and Canada.

摘要: The proportion of overweight and obese adults in the United States Canada has increased over past decade, but temporal trends body mass index (BMI) weight gain on antiretroviral therapy (ART) among HIV-infected have not been well characterized. We conducted a cohort study comparing North America AIDS Cohort Collaboration Research Design (NA-ACCORD) to National Health Nutrition Examination Survey (NHANES) controls matched by sex, race, age period 1998 2010. Multivariable linear regression assessed relationship between BMI year ART initiation, adjusting for age, baseline CD4(+) count. Temporal were using generalized least-squares model further adjusted HIV-1 RNA first regimen class. A total 14,084 patients from 17 cohorts contributed data; 83% male, 57% nonwhite, median was 40 years. Median at initiation 23.8 24.8 kg/m(2) 2010 NA-ACCORD, percentage those (BMI ≥30 kg/m(2)) 9% 18%. After 3 years ART, 22% individuals with normal (18.5-24.9 had become 25.0-29.9 kg/m(2)), 18% obese. white women higher after as compared age-matched NHANES (p = 0.02), while no difference observed men or non-white controls. high prevalence obesity we ART-exposed may contribute health complications future.
