摘要: Background: The management of splenic injury resulting from blunt trauma in adults is controversial, with an increasing trend towards non-operative and conservation the spleen. A retrospective review was performed on adult patients treated a single institution for attempt to identify factors important selecting appropriate option predicting success that option. Methods: Associated injuries (standardized using Injury Severity Scores), clinical signs at presentation, computed tomographic grading injury, transfusion requirements were documented. Statistical analysis non-parametric Mann–Whitney, Chi-squared, Kolmogorov–Smirnov multivariate logistic regression tests. Results: Eighty-five identified. Non-operative used 39 patients, 14 splenectomy 32 patients. mean Score significantly lower group. Computed not found correlate well intraoperative findings. Transfusion failed four patients; two had continued bleeding, required surgery other intra-abdominal injuries. Overall mortality 7%. There one death group, unrelated second laparotomy persistent bleeding. five deaths only which related surgery. Conclusion: Management remains controversial. decision pursue rather than or depends individual merits each case. conservation, particularly younger, stable organ injury.
