摘要: OBJECTIVE: To examine risk factors for nursing home placement in a population-based dementia cohort. METHODS: The Mayo Clinic Medical Records linkage system was used to identify all patients with onset of between 1980 and 1984. patient group included 314 cases who met DSM-III-R criteria dementia, including 220 were community dwelling at onset. All followed until death. A control 323 did not, any point, meet criterion dementia. groups initially matched on age, gender, year initial registration. Demographic, medical, social, functional predictors examined as static time-dependent the community-dwelling subgroups, using stepwise Cox regression modeling. RESULTS: Of patients, 282 took residence licensed skilled homes least 6 weeks, suggestive custodial care, some point during course their illness. In group, 162 people required placement. Within controls, predictor variables time being divorced, living townhome, apartment or assisted apartment, change Charlson comorbidity score, amount daily assistance required. sample, seven eventually determined be associated These total number years education, age single, retirement supervised onset, required. CONCLUSIONS: Cumulative incidence 90% cohort 50% controls. Certain seem impact older persons, whether they have not. Among these are settings, increasing scores, need assistance. additional may specific is which seems provide protective effect. important covariates clinical studies that include an outcome variable.
