Mechanism of Acute Ischemic Stroke in Patients with Severe Middle Cerebral Artery Atherosclerotic Disease

摘要: Background Intracranial atherosclerotic disease (ICAD) is one of the most common causes ischemic stroke worldwide. Although pathogenesis cerebral infarct in ICAD has been reported from autopsy series, mechanism not well known. This study used baseline perfusion imaging and diffusion-weighted (DWI) or computerized tomography (CT) to help identify involving middle artery (MCA). Methods We retrospectively reviewed CT magnetic resonance (MR) studies MR scans patients with severe symptomatic MCA stenosis. Perfusion were classified according stage deficit, acute patterns categorized as borderzone, cortical, perforating infarcts DWI noncontrast CT. Results Fifteen included this analysis. All 15 had some type borderzone infarct. Six only, 4 cortical infarcts, 5 infarcts. Thirteen deficits. Conclusions In ICAD, multifactorial, but hemodynamic insufficiency plays a significant role. finding important selecting subgroup who may benefit revascularization.
