Sensitivity of the dipstick in detecting bacteremic urinary tract infections in elderly hospitalized patients.

摘要: Background The sensitivity of the dipstick in elderly patients with a suspected urinary tract infection (UTI) is unclear because inclusion urine contamination or asymptomatic bacteriuria previous studies. Methods We selected consecutive aged 65 years older hospitalized internal medicine departments bacteremic UTI (same organism blood and cultures) minimizing misclassifications. The false positive rate was determined negative culture results. A test result trace leukocyte esterase and/or nitrite positivity. Bacteriuria growth at least 105 colony-forming units per milliliter urine. Results Of 20,555 patients, 228 had UTI, 4069 result. 96.9% (95% CI—93.7–98.6) 42.4% CI, 41.0–43.8) those result. Conclusions In urinalysis highly sensitive, much higher than reported previously studies UTIs elderly. It whether observed high due to exclusion spectrum bias. Studies clinical utility/disutility using rule out are warranted.
