Recurrent varicose veins after surgery: a new appraisal of a common and complex problem in vascular surgery.

摘要: Objective. To assess the true incidence, reflux patterns and mechanisms responsible for recurrent varicose vein disease according to current definitions guidelines. Patients methods. Ninety-three patients (69 female, 24 male, mean age: 48 years) were prospectively evaluated pre- postoperatively (1 month 5 years), using clinical colour duplex examination of both lower limbs. The CEAP score its modification recurrence (REVAS) used classification. Results. In 113 operated limbs, 28 (25%) found have a recurrence, 20 which symptomatic (20/28, 72%). However, in this group, severity decreased significantly from 6.5 (SD 3.1) 5.2 2.8) (p<0.001, paired t-test). The correlation between type cause revealed: (1) varices eight limbs (8/28, 29%), primarily caused by neovascularisation, (2) new veins as consequence progression seven (7/28, 25%), (3) residual three (3/28, 11%) mainly due tactical errors (e.g. failure strip GSV), (4) complex 10 (10/28, 36%). In with 42 sources venous identified: 19 sites (15% limbs), 13 neovascularisation (11.5% six resulted failures (5.3% limbs) four technical (3.5% limbs). Conclusions. This study shows that after surgery is not uncommon. condition most affected remains improved. Progression are more than half recurrences. Rigorous evaluation assiduous surgical technique might reduce failures.
