摘要: Importance The efficacy and safety of primary intraocular lens (IOL) implantation during early infancy is unknown. Objective To compare the visual outcomes patients optically corrected with contact lenses vs IOLs following unilateral cataract surgery infancy. Design, Setting, Participants Infant Aphakia Treatment Study a randomized clinical trial 5 years follow-up that involved 114 infants congenital cataracts at 12 sites. A traveling examiner assessed acuity age 4.5 years. Interventions Cataract or without IOL implantation. Contact were used to correct aphakia in who did not receive IOLs. was determined through random assignment. Main Outcomes Measures HOTV optotype age. Results median logMAR significantly different between treated eyes 2 treatment groups (both, 0.90 [20/159]; P = .54). About 50% both had less than equal 20/200. Significantly more group least 1 adverse event after (contact lens, 56%; IOL, 81%; = .02). most common events reproliferation into axis, pupillary membranes, corectopia. Glaucoma/glaucoma suspect occurred 35% 28% ( = .55). Since initial surgery, have additional 21%; 72%; Conclusions Relevance There no significant difference operated children underwent those left aphakic. However, there intraoperative procedures group. When operating on an infant younger 7 months cataract, we recommend leaving eye aphakic focusing lens. Primary should be reserved for where, opinion surgeon, cost handling would so burdensome as result periods uncorrected aphakia. Trial Registration clinicaltrials.gov Identifier:NCT00212134
