Primary aldosteronism associated with subclinical Cushing syndrome

摘要: Background: Recently, it has been reported that the incidence of primary aldosteronism (PA) among patients with hypertension is much more frequent than previously reorted. Aim: In present study, we investigated frequency and features PA associated subclinical Cushing syndrome (SCS). Material methods: Subjects included consecutive (no.=39) who were diagnosed as performed adrenal venous sampling between 2003 2011 in our institute. Results: 39 subjects PA, 29 operated 5 cases (12.8%) showed no suppression low-dose dexamethasone test. Four them demonstrated to be SCS, one was overt (CS). Post-operatively, 3 received replacement therapy hydrocortisone, while others did not. Pathological findings indicated diagnosis aldosterone-producing adenoma 4 idiopathic hyperaldosteronismin case CS. all cases, immunohistochemical analysis immunoreactivities both 3βHSD P450c17 adrenocortical tumors, marked cortical atrophy zona fasciculata reticularis, decreased dehydroepiandrosterone sulfotransferase expression, hypothalamo-pituitary-adrenal axis indicating autonomous secretion cortisol from tumor. Conclusions: The study suggests frequently SCS prevalence 10%, justifying routine examinations for cases.
