Educational and Organizational Interventions to Improve the Usefulness of Clinical Pharmacological Advice for Personalized Drug Dosing Based on Therapeutic Drug Monitoring.

摘要: This study was aimed at increasing the clinical usefulness of pharmacological advice (CPA) for personalized drug dosing based on therapeutic monitoring (TDM). Educational and organizational interventions focused improving knowledge pharmacology among hospital healthcare workers reducing incidence errors throughout process were planned. After a pre-interventional period risk assessment, different list forms types error occurring in various phases (Phase 1, request CPA blood sampling TDM; Phase 2, sample delivery to check CPU; 3, TDM execution production) created. In interventional period, collected daily educational programmes carried out. The pre-intervention rate 19.5%, resulted significantly higher requests coming from medical wards compared with those surgical or ICUs (26.0% versus 10.5% 13.7%, p < 0.001). programme trained 303 nurses 145 physicians. Afterwards, percentage progressively dropped (15.5% 2nd trimester; 12.3% 3rd one; 4th one). adopted strategy significant improvements which may be useful both improve quality patient care reduce waste costs.
