摘要: Background and Objectives Plasma platelet concentrates are disproportionately implicated in transfusion-related acute lung injury (TRALI). Platelet-derived pro-inflammatory mediators, including soluble CD40 ligand (sCD40L), accumulate during storage. We hypothesized that contamination induces sCD40L generation causes neutrophil [polymorphonuclear leucocyte (PMN)] priming PMN-mediated cytotoxicity. Materials Methods was untreated, centrifuged (12 500 g) or separated from leucoreduced whole blood (WBLR) prior to freezing. Platelet counts concentrations were measured 1–5 days post-thaw. The plasma assayed for PMN activity used a two-event vitro model of human pulmonary microvascular endothelial cell (HMVEC) cytotoxicity. Results Untreated contained 42 ± 4·2 × 103/μl platelets, which generated accumulation (1·6–eight-fold vs. controls). Priming HMVEC cytotoxicity directly proportional concentration. WBLR centrifugation reduced contamination, abrogating the potential. Conclusion stored may contribute TRALI. reduction is potentially first TRALI mitigation effort manufacturing.
