摘要: Objective To assess the diagnostic accuracy of cervical conization in women with adenocarcinoma situ and to determine whether a select group could be managed by alone without hysterectomy. Methods We retrospectively reviewed 40 cases diagnosed on conization. Results Cervical revealed 15 women. Twenty-five had coexisting squamous dysplasia (23) or microinvasive cell carcinoma (two). Twenty-two underwent hysterectomy after cone biopsy. Adenocarcinoma was detected specimen one 12 uninvolved margins, versus seven ten involved margins (P = .006); two these also foci invasive specimen. Conization only treatment for 18 selected margins; all were relapse-free median interval 3 years (range 1.5-5). Conclusions Women who have positive are at high risk residual moderate occult adenocarcinoma; expectant management is not warranted. However, biopsy can reliably guide subsequent therapy. Selected young desire preservation fertility probably alone, but further study required establish safety this approach.
