Tracheal and Main Bronchial Disruptions After Blunt Chest Trauma: Presentation and Management

摘要: Tracheobronchial disruption is one of the less common injuries associated with blunt thoracic trauma. This injury can be life threatening, however, and failure to diagnose it early lead disastrous acute or delayed complications. Nine cases tracheobronchial in setting nonpenetrating trauma were seen at four Los Angeles centers between 1980 1987. Mechanism injury, presentation, diagnosis, management these patients reviewed. Disruptions involved trachea 3 patients, right bronchus 5 left 2 patients. disruptions occurred settings high-energy impact-type more likely have than they occur alone. Common presenting signs included subcutaneous emphysema, dyspnea, sternal tenderness, hemoptysis. Radiographic findings most commonly pneumothorax, pneumomediastinum, clavicle rib fractures. Rigid bronchoscopy fiberoptic both highly accurate methods for diagnosis but only hands trained cardiothoracic surgeons. Delay increased likelihood postoperative
