Diferencias entre los canceres de mama diagnosticados clinicamente y los detectados en un programa de cribado

摘要: OBJETIVOS: El objetivo principal fue determinar si los canceres de mamadiagnosticados clinicamente tenian caracteristicas histologicas y biologicasdiferentes a que se diagnostican en un programa cribado. Los objetivossecundarios fueron comparar factores riesgo el tratamiento recibido porlas pacientes diagnosticadas partir cribado lasdiagnosticadas clinicamente.METODOLOGIA: Estudio casos controles. Se incluyeron 311 pacientescon diagnostico histologico cancer mama edad entre 45 65 anos setrataron Servicio Oncologia Medica del Hospital Universitario La Fe deValencia 1999 2003. En funcion metodo (cribado oclinico) establecieron 2 grupos: grupo precoz donde seincluyeron 158 (DP) clinico (DC) 153 pacientes.RESULTADOS: Las tenianmayor porcentaje carcinomas in situ (DP 9.5%, DC 2.62), tumores invasivosmenores 10 mm 25.9%, 8.1%), menor afectacionganglionar axilar 75.9%, 55.6%) mas diferenciados (DP38%, 24.8%). mayorporcentaje receptores estrogeno 87.3%, 79.1%), progesterona (DP84.3%, 76.7%) expresion bcl2 75.3%, 62.9%). Laspacientes procedentes trataron con cirugia conservadora (DP83.2%, 62.5%) hormonoterapia 92.3%, 82.9%) menosquimioterapia 44.3%, 78.4%).CONCLUSIONES: DP tienen mayor carcinomasno invasores este son tamano ytienen menos afectacion ganglios axilares. diagnosticados enel agresividad elde (grado histologico, receptor progesterona, debcl2). mamografico permite utilizar tratamientos quirurgicos menosagresivos adyuvante quimioterapia sistemica, no asicon hormonoterapia.
