Abstract:  This retrospective case series reviews our center’s experience with sirolimus a CNI as alternative therapy for the treatment PTAH. It also characterizes (Tregs) LT recipients PTAH who had received or were receiving retrospectively identified (n = 12). Liver enzymes, immunohistochemistry, histology compared all 12 patients. Immunophenotyping Tregs peripheral blood mononuclear was performed on conventional CNI, azathioprine ± prednisone (CT) (n = 11), sirolimus, CNI ± prednisone (n = 8), without (n = 25), pre-transplant patients (n = 5). Severity necro-inflammatory changes markedly improved sirolimus. Treg frequency number significantly lower CT to (i) those (p = 0.002 p = 0.01, respectively), (ii) (p = 0.07 p = 0.009, respectively). higher under (p = 0.027). Sirolimus addition is successful reversing associated circulating Tregs.
