摘要: Background and purpose Standard treatment for massive hemorrhage in dogs is infusion of whole blood or packed red cells with fresh frozen plasma if not available. Although most collected using a citrate-based anticoagulant, knowledge citrate's relevant non-anticoagulant effects widespread. Citrate's anticoagulant activity achieved through chelation divalent metal cations (e.g., magnesium, calcium), which may exacerbate cardiovascular metabolic insults attributable to hemorrhage. Methods Blood pressures, gas tensions, metabolites, electrolytes; myocardial contractility; cardiac output; left cranial descending circumflex coronary artery flows were measured 21 anesthetized after was induced by collection into citrated reservoir mean arterial pressure 45 mm Hg approximately 60 min (until lactate concentration 7.0 mmol/L), followed 1-h transfusion 2 h maintenance. Results Arterial ionized calcium concentration, total peripheral resistance, function decreased significantly during All aforementioned responses but continued decrease the initial 20 transfusion, then began recover. Total resistance end-systolic elastance only factors related concentration. Conclusion Transfusion alter negatively affecting vascular function.
