摘要: As the burden of chronic and complex disease grows, there is an emphasis on programs that enhance quality care within primary care. The Connecting Care in Community (CCC) program example implementation integration. This qualitative study sought to explore experiences general practice staff face managing clients with issues, their perceptions contribution CCC this Seventeen from 11 practices throughout Illawarra/Shoalhaven region participated semistructured interviews. Five main themes emerged: (1) awareness program; (2) varying exposure value placed (3) 'busyness' role confusion; (4) communication information sharing; (5) need for education knowledge local resources. If policymakers healthcare organisations can gain a better understanding staff, they will be able design implement best meet needs providers are attempting integrate.
