摘要: Abstract Study objective We describe errors occurring in a busy ED. Methods This is prospective, observational study of reported at an academic emergency department (ED) with 100,000 annual visits. Trained personnel interviewed all ED staff direct patient contact, during and the end every shift, by using standardized data sheets. Results One thousand nine hundred thirty-five patients registered 7-day period summer 2001. Four error reports were generated, identifying 346 nonduplicative (18 per 100 patients; 95% confidence interval [CI] 15.9 to 20.0). Forty percent nurses, 25% providers, 19% clerical staff, 13% technicians orderlies, 3% multiple reporters. Errors for hours worked similar groups (5.5; CI 5.2 5.9). categorized as 22% diagnostic studies, 16% administrative procedures, pharmacotherapy, documentation, 12% communication, 11% environmental, 9% other.
