Respiratory and Gastrointestinal COVID-19 Phenotypes in Kidney Transplant Recipients.

摘要: Background Coronavirus infectious disease 2019 (COVID-19) pandemic has posed at risk the kidney transplant (KT) population. We describe clinical pictures, factors for death, and chances to recovery in a large cohort of KT recipients with COVID-19. Methods Inclusion Spanish prospectively filled registry was allowed cases confirmed Outcomes were assessed as in-hospital mortality or recovery. Results The study population comprised 414 patients. Fever, respiratory symptoms, dyspnea most frequent COVID-19-related 81.4% them had pneumonia. More than one-third patients showed digestive symptoms diagnosis, combinations nausea, vomiting, diarrhea. Most hospitalized, 12.1% intensive care units, 17.6% needed ventilator support. Treatment COVID-19 included frequently hydroxychloroquine, azithromycin, high-dose steroids, lopinavir/ritonavir, tocilizumab. After mean follow-up 44 days, fatality rate 26.3%. Pneumonia without gastrointestinal associated 36.3% (respiratory phenotype), pneumonia 5.3% (gastrointestinal phenotype). mixed phenotype an intermediate 19.5% (mixed Multivariate Cox regression analysis that age independently whereas Conclusions is among Advanced are main features high-mortality rate. Gastrointestinal more benign course lower mortality.
