摘要: Because of the high mortality associated with a diagnostic open lung biopsy in patients on mechanical ventilatory support, clinical benefits this procedure remain controversial. To assess surgical results and usefulness biopsies performed subset patients, records 27 who underwent between January, 1987 December, 1992 were reviewed. All intubated for respiratory failure unknown origin treated antibiotics and/or steroids prior to biopsy. There 13 men 14 women mean age 51 +/- 4.7 years (3 mos. 82 years). The average duration support was 9 3.7 days (1 98 days). Nonoperative invasive procedures 19 (70%). operative approach an anterolateral thoracotomy using linear stapler: right 10 (37%), left 17 (63%). A specific histologic diagnosis established all patients. Based findings, change treatment unnecessary (33%), dose corticosteroids increased 7 (26%), initiated 6 (22%), new antibiotic added 5 (19%). Perioperative complications occurred 11 (40%). Prolonged air leak most common complication (6/11 patients). Fourteen (52%) died within 30 procedure.(ABSTRACT TRUNCATED AT 250 WORDS)
