摘要: Treating chronic illness is becoming increasingly team-based, yet most team building concepts relate to the as a dichotomous unit: medical and patient. Patients are expected be “engaged” “activated” members of team, there gap in knowledge regarding how patients effectively integrate into team. Exacerbating complexity patient member multifaceted, ever-changing mood state with cooccurring mental health conditions. The field well aware correlations between illnesses. purpose this study investigate, from patient’s perspective, what factors influence sense connectedness care these mediate positive behaviors relative self-care. Findings mixed-methods approach showed that level perceived confidence self-care was mediated by presence diagnosis. Mixed-methods data also high levels their had no statistical connection increased
