Aplicación del Shock Index como predictor de hemorragia en el paciente politraumático

摘要: Resumen Introduccion Las constantes vitales detectan la presencia de hemorragia al perder grandes cantidades sangre, lo que comporta una gran morbimortalidad. El Shock Index (SI) es un parametro detecta el sangrado con puntos corte 0,9. objetivo este estudio valorar si punto de ≥ 0,8 mas sensible para detectar oculto, permitiendo iniciar maniobras terapeuticas precoces. Metodos Estudio analitico validacion del SI incluye pacientes politraumatizados graves mayores 16 anos. Se registran y escalas predictivas sangrado: SI, Assessment of Blood Consumption score Pulse Rate Over Pressure score. analiza relacion 5 marcadores predictivos necesidad transfusion masiva, embolizacion angiografica, control quirurgico, muerte por shock hipovolemico «sangrado activo» (presencia menos uno los 4 anteriores en paciente). Resultados Recogida prospectiva datos 1.402 durante 10 Injury Severity Score medio fue 20,9 (DE 15,8). Hubo mortalidad 10%. 0,73 0,29). En total presentaron 18,7% serie. 0,87, mientras las estaban dentro normalidad. area bajo curva ROC 0,749. Conclusiones corte ≥ 0,8 aquel corte ≥ 0,9 permite reanimacion precoces oculto.
