Temporal changes in the causes of aortic stenosis: a surgical pathologic study of 646 cases.

摘要: Among 646 patients with pure aortic stenosis who underwent valve replacement at our institution between 1981 and 1985, the three most frequent causes were calcification of congenitally bicuspid valves (38%), degenerative (senile) tricuspid (33%), postinflammatory (presumably rheumatic) fibrosis (24%). 324 younger than 70 years age, calcified observed in 50%. In contrast, among 322 age or older, accounted for 48% stenotic valves. During 5 study, relative frequency disease decreased from 30% to 18%, that 37% 33%. increased 46%. Consequently, is currently common cause undergoing institution. This finding may be related changes life expectancy general population, alterations patient referral practices, an willingness surgeons operate on older patients. Regardless cause, temporal etiologic factors indicate a potential source increasing health-care costs elderly population.
