Building laboratory capacity to support HIV care in Nigeria: Harvard/APIN PEPFAR, 2004–2012

摘要: Introduction: From 2004–2012, the Harvard/AIDS Prevention Initiative in Nigeria, funded through US President’s Emergency Plan for AIDS Relief programme, scaled up HIV care and treatment services Nigeria. We describe methodologies and collaborative processes developed to improve laboratory capacity significantly a resource-limited setting. These methods were implemented at 35 clinic locations. Methods: Systems established modified optimise numerous processes. These included strategies selection management, equipment reagent procurement, supply chains, renovations, maintenance, electronic data management, quality development programmes trainings. Results: Over eight-year laboratories supported 160 000 patients receiving HIV care delivering over 2.5 million test results, including regular viral load quantitation. External assurance systems CD4+ cell count enumeration, blood chemistries load monitoring. Laboratory platforms were improved standardised use of point-of-care analysers was expanded. Laboratory training workshops toward increasing staff skills improving overall quality. Participation World Health Organisation-led African quality improvement system resulted significant gains measures five laboratories. Conclusions: Targeted implementation processes, during simultaneous scale-up setting, can elicit meaningful capacity. physical laboratory environment, develop staff, create improvements reduce costs and increase are available future health strengthening programmes. We hope that employed may inform encourage other laboratories settings.
