Use of epidemiological cutoff values to examine 9-year trends in susceptibility of Aspergillus species to the triazoles.

摘要: In the absence of clinical breakpoints, epidemiological cutoff values (ECVs) have been established to distinguish wild-type (WT) isolates Aspergillus spp. from those that may harbor resistance mutations. Recently, CLSI has developed ECVs for triazoles (itraconazole, posaconazole, and voriconazole) common species. We applied triazole 1,789 collected 63 centers worldwide 2001 2009 determine frequency non-WT strains each Temporal trends were evaluated fumigatus flavus over 9-year period drug. The collection included 1,312 A. fumigatus, 235 flavus, 162 niger, 64 terreus, 15 versicolor. Using ECVs, percentages itraconazole, voriconazole, respectively, as follows: (2.0%, 3.5%, 1.4%), (0.8%, 5.1%, 1.7%), niger (17.3%, 3.7%, 0.6%), terreus (0.0%, 1.6%, 3.2%), versicolor (6.3%, 0.0%, 0.0%). Among 49 which itraconazole MICs >2 μg/ml, posaconazole voriconazole greater than 14 12 isolates, respectively. ranged 1.1 5.7% 0.0 1.6% 0.7 4.0% itraconazole. There was no consistent trend toward decreased susceptibility any or time. Decreased among observed extended-spectrum varied by species study period.
