摘要: Helicobacter pylori (H. pylori) infection is a major cause of symptomatic gastritis and peptic ulcers in the Kenyan population. Persistence H. its medical complications continue to be problem. Treatment duration usually for one or two weeks. Studies developed countries have shown equal increased efficacy with week triple therapy treatment, but no study has been done establish this Kenya. The main aim was compare eradication adults at Nairobi gastroenterology clinic. This single-blind randomized clinical trial conducted Centre Clinical Research, Kenya Medical Research Institute, - Patients aged 18 years above, referred clinic endoscopy, were interviewed, asked participate consent acquired from them. 479 patients went through rapid urease testing on endoscopy which 253 tested negative 226 positive pylori. prevalence rate 47%. 150 receive either esomeprazole 20 milligrams, amoxicillin 1 gram, clarithromycin 500 milligrams twice daily followed by plus placebo day (EAC1, n = 76), gram weeks (EAC2, 74). A cure check stool antigen (Meridian Bioscience) performed 4 after conclusion therapy. Quantitative data analyzed using SPSS computer program. Bivariate multivariate analysis measure strength association between exposures theoutcomes. 40.2% 112 patients, who completed follow up, found following – 59.8%. eradicated 66.1% EAC1 compared 53.6% those EAC2 per protocol analysis. There statistical difference noted (P>0.05). Both treatments similarly well tolerated unexpected safety concerns. side effects abdominal pain headache both treatment groups. significant occurrence particular effect groups presence return symptoms end bivariate (P<0.05). final logistic regression model incorporated predictors (P<0.05): Age Return comparison presence. For every unit increase age, an individual 5% less likely test older there An showing recurring 3.31 times more without recurrence symptoms. In population, seven as effective fourteen eradication. However, recommended it good results cost-effective. tolerated. It may necessary levels drug resistance before retreatment will help determine type drugs use treatment.
