Validity of criteria for hospital admission in exacerbations of COPD.

摘要: SummaryAims To validate a previously developed set of explicit criteria for the appropriateness hospital admission among these patients using RAND/UCLA Appropriateness Methodology (RAM). Methods We conducted prospective cohort study experiencing symptoms COPD exacerbation seen in emergency departments (ED) 16 hospitals belonging to Spanish National Health Service. Sociodemographic and clinical variables needed assess were recorded. Main outcomes mortality, severe evolution, complications at follow up, three patient-reported measures: dyspnoea level, capacity physical activity perceived health status. Results Appropriately admitted more likely die (6.70% vs. 2.68%, p = 0.0102) than inappropriately patients, develop evolution (27.09% 6.08%, p < 0.0001) (18.72% 11.92%, p = 0.0244). Among discharged no significant differences observed outcomes. All exhibited worse after exacerbation, but changes appropriately less patients. Conclusion Our identified condition ED arrival who benefit from terms mortality evolution.
