摘要: Objective The current study sought to evaluate and replicate the severe impairment profile (SIP) of Word Memory Test (WMT) in patients referred for dementia evaluations. Method sample consisted 125 a neuropsychological evaluation at large Veterans Affairs Medical Center. Patients were assigned Clinical Dementia Rating (CDR) by blind raters, classified according their performance on validity testing. Subsequent chart reviews conducted help more accurately determining presence memory likely due an underlying dementing process versus poor effort/task engagement. Results In our sample, 51% failed easy WMT subtests 93% these obtained SIP. rates failure generally coincided with both severely impaired CDR ratings, as well delayed composite scores. Upon review, it was determined that there significant portion classification errors using SIP, positive posttest probability based having SIP being 65% opposed 28% negative result. Conclusions Our findings suggest does not appear function effectively mixed where is increased potential secondary gain. Additional concern expressed regarding overall likelihood obtaining subsequent inferential decisions related Future research should examine optimal cut scores or alternative methods classifying different clinical contexts patterns impairment.
