Empyema Thoracis : Therapeutic Management and Outcome

摘要: Study objective We evaluated treatment and outcome of patients with thoracic empyema at a teaching institution. Design setting Retrospective chart review over 44-month period university hospital. Patients measurements Charts hospital discharge diagnosis were reviewed. Age, symptoms, alcohol use, etiology, culture results, number loculations, date success each procedure, length stay, status recorded for patient. Success recovery time, time between procedures, total hospitalization compared procedures subgroups. Results from 43 Twenty-four (56%) cases parapneumonic empyemas. Forty (93%) had symptoms attributable to their empyema, fever being the most common (65%). Seventy-nine needed treat (1.84 per patient). rates ranged 11% (3/27) tube thoracostomy 95% (21/22) decortication (p=0.0001). Delay averaged 6.2 ± 1.1 (mean SEM) days first second procedure (n=27), 10.4 5.1 third (n=8). Mean after successful intervention 9 19.3 depending on (p=NS). Comparisons multiloculated uniloculated empyemas, nonparapneumonic proven biochemically empyemas showed no significant difference in or stay. Conclusion Multiple therapeutic options exist empyema. Optimal therapy requires selection appropriate patient early postprocedure imaging avoid inordinate delays interventions.
