摘要: Purpose. The implementation of a multidisciplinary approach to systematically decrease medication errors through the use observation methodology and deployment electronic administration records (EMARs) bar-coded-medication (BCMA) is described. Summary. For consistent reliable data collection, direct-observation technique was used. measurement using process occurred in two phases—preimplementation postimplementation. Three inpatient nursing units participated. control group 20-bed cardiac telemetry unit. Intervention 1 also 2 36-bed medical–surgical During first phase study, all three study groups participated evaluating associated with manual five-day record (MAR). A total 188 were reported. pharmacy, nursing, information services departments collaborated on design EMAR BCMA systems. systems implemented one unit August 2003, full by July 2004. second continued MAR without change process. measured evaluate technology. accuracy rate before 86.5%; after BCMA, rose 97%. Conclusion. used monitor 54% reduction observed following multidisciplinary, collaborative safety.
