Feasibility and performance of a patient-oriented discharge instruction tool for heart failure.

摘要: Background The provision of patient-centred discharge instructions is a pivotal goal for improving quality care patients with heart failure (HF) during transitions. We tested the feasibility and performance novel instruction tool co-designed adapted HF; patient-oriented summary (PODS-HF) aim communication, comprehension adherence to instructions. Methods An iterative process was used adapt implement an existing patient HF (PODS-HF). A mixed methods approach then explore experience, using pre–post study design among eligible admitted over 6-month period. Outcome measures included: documentation instructions, locally derived Average Discharge Score (ADS) based on inclusion in nine key areas, satisfaction understanding at 72 hours 30 days determined follow-up phone calls. Results 19 were enrolled. ADS increased by 68% more consistent documentation. Patient remained high. Patients provided PODS-HF reported receiving written information about related signs symptoms watch (two out five usual group vs seven group; p=0.045). also felt confident manage their own health 30-day diet exercise improved while reducing need unscheduled visits. Quantitative results supported themes identified calls, namely, utility importance call. Conclusion feasible delivery HF. individual benefits clarification reinforcement made calls this remains be clarified.
