摘要: Resume Les douleurs neuropathiques sont sous-estimees et souvent mal traitees. L’echelle DN4 est particulierement utile a leur identification car elles relevent d’une prise en charge specifique pharmacologique non pharmacologique. mecanismes physiopathologiques impliquent une hyperexcitabilite des voies nociceptives ou diminution controles inhibiteurs descendants qui vont etre la cible traitements pharmacologiques. molecules de premiere ligne les antidepresseurs (tricycliques mixtes recapture serotonine noradrenaline) antiepileptiques α2δ canaux calciques. Ces n’auront pourtant qu’une efficacite partielle sur douleur neuropathique. La strategie therapeutique s’appuie objectifs raisonnables debutant par monotherapie adaptee aux symptomes comorbidites du patient, augmentee paliers. L’adhesion patient au contrat fixe est indispensable implique information claire complete ainsi que compte retentissement professionnel, social familial. En cas d’inefficacite, un changement traitement association peuvent realises. Certaines indications justifient specifique. chroniques resistantes seront adressees centre specialise. De nouvelles l’etude prises pharmacologiques doivent evaluees.
