Significance of serum procalcitonin as biomarker for detection of bacterial peritonitis: a systematic review and meta-analysis

摘要: Bacterial peritonitis is serious disease and remains a diagnostic challenge for clinicians. Many studies have highlighted the potential usefulness of procalcitonin (PCT) identification bacterial peritonitis, however, overall value PCT unclear. Therefore, we performed meta-analysis to assess accuracy detection peritonitis. We systematic searched in MEDLINE, EMBASE, SCOPUS, China Biology Medicine Database (CBM), National Knowledge Infrastructure (CNKI) Cochrane databases trials that evaluated role Sensitivity, specificity other measures were pooled using bivariate random effects models. Eighteen involving 1827 patients included present meta-analysis. The sensitivity serum diagnosis 0.83 (95% CI: 0.76–0.89) 0.92 0.87–0.96), respectively. positive likelihood ratio was 11.06 6.31–19.38), negative 0.18 0.12–0.27) odds (DOR) 61.52 27.58–137.21). area under receiver operating characteristic curve (AUROC) 0.94. Use common cut-off could improve DOR 75.32 AUROC 0.95. Analysis seven measured C-reactive protein (CRP) indicated more accurate than CRP Our results indicate determination relatively sensitive specific test However, with regard methodological limitations significant heterogeneity, medical decisions should be based on both clinical findings results.
